$199.00 for each new registration (License good for 5 years) 
$99.00 for each renewal (For an additional 5 years)

How to Register
Please follow these steps to register.  
Step 1. Download the registration form PDF. (You need to have the Adobe Acrobat Reader plug-in installed to view the PDF. Acquire the FREE plug-in by clicking here).
Step 2. Print out the form and fill it out completely.
Step 3. Submit the completed form to us.

Payment
Payment is due once we process your registration. If paying by check, please make check payable to PLAIT. If paying by credit card, please fill out Credit Card Authorization Form. Upon receipt of your payment, a receipt will be e-mailed to you confirming your registration in the course. 
Refund Policy
1. Master Technician registration fees are refundable for cancellations made fourteen (14) days prior to the training course date. 2. Any cancellations made less than fourteen (14) days before the course date will be subject to a late cancellation fee of $85.00 for new registrations and $45.00 for renewals. 3. Registrants who do not attend the course will be considered no-shows and are liable for the entire course
